What this template is for
Use it to ask the PCP practice to review and send the referral. It is a general office template, not a clinician-signed referral, an insurer-specific form or a prior authorization approval.
If the PCP, health plan or IPA requires its own form or portal, follow those instructions instead. Confirm the current requirements with the receiving office before sending.

- Verify the destination fax and your return fax. Review every completed field and attachment before sending through your office’s approved fax service.
- Keep the request and delivery record in the appropriate office system. Assign a staff owner and next follow-up date; review the PCP’s actual response before deciding what happens next.
