Can controlled substance prescriptions be faxed?
Yes, for Schedule III, IV, and V controlled substances. Under DEA regulations (21 CFR 1306.21), a facsimile of a prescription may serve as the original for Schedules III–V. Schedule II controlled substances (opioids, stimulants) generally require an original written prescription, with limited exceptions for long-term care facilities and hospice patients. Always verify your state's specific requirements, as some states impose additional restrictions.

What information must a faxed prescription include?
Requirements vary by state and substance schedule, but generally: patient name and address, date, drug name, strength and quantity, directions for use, prescriber name, address, DEA registration number (for controlled substances), and prescriber signature. For faxed controlled substance prescriptions, federal law requires the prescriber's manual signature on the original, which is retained by the prescriber.
Can a patient fax their own prescription to a new pharmacy?
For non-controlled substances, patients sometimes fax their own prescriptions when transferring pharmacies — though the receiving pharmacy may request confirmation directly from the prescriber. For Schedule III–V controlled substances, the fax must come from the prescriber, not the patient. Check with the receiving pharmacy on their specific policy.
How do I fax a prior authorization to an insurance company?
Most insurers publish a prior authorization fax number on their provider portal or on the back of the member's insurance card. Complete the PA form (often available from the insurer's website or your EHR), attach supporting clinical documentation, and fax to that number.
